AHSAA Required Athletic Forms
Before a student may participate in any athletic practice, workout, or competition, all required forms mandated by the Alabama High School Athletic Association (AHSAA) must be completed and submitted. These forms help ensure the safety and eligibility of all student-athletes and include required medical, consent, and eligibility documentation.
Parents and students are encouraged to complete all required forms as early as possible to avoid delays in participation. Students will not be permitted to participate until all required paperwork has been received and approved.
The following forms should be completed digitally within the student's DragonFly Max account:
- Concussion Form
- Student Release Form
- Student/Parent Sudden Cardiac Arrest Form
Additionally, students are required to have a CURRENT PHYSICAL on file. Tuscumbia City Schools partners with Helen Keller to provide free yearly physicals in May of each year. Students may also have a physical completed at a doctor's office. The Preparticipation Physical Exam Form should be completed by a doctor and uploaded to the student's DragonFly Max account. Students should also turn in the paper copy of the physical to the head coach of their sport.
If you have questions about the required forms or the submission process, please contact the Deshler High School Athletic Department.
